Helped to develop the palliative home program, visited patients and case managed their care along with palliative provider.  Worked to coordinate care among several agencies in Bon Secours

Helped develop and implement an Advanced Illness Management Program (AIM) now the home based palliative program, specializing in the care and focus of the physical, emotional and spiritual needs of people facing advanced illness.
