Overview
SCCAPE is a 4-day, multidisciplinary course for physicians, physician assistants, nurses, social workers, psychologists, pharmacists, community health providers, patients, and their families designed to provide the knowledge and skills needed for participants to understand and optimize the role of a sickle cell disease health navigator (SCD HN) or work in a Sickle Cell Disease medical care team. This interprofessional conference prepares allied health professionals, community-based staff, and volunteers to work together to improve patient outcomes.

Objectives
  1. Describe sickle cell disease (SCD) pathology, complications, treatments and the impact on patients’ general health and lifestyle.
  2. Summarize the newer SCD modifiers and emerging curative therapies.
  3. Identify the impact of mental health concerns in SCD care and discuss behavioral health screening tools and strategies to assist patients.
  4. Discuss how a sickle cell medical home is developed and describe the critical components of a program including staffing, financing, daily operations and monitoring.
  5. Describe an effective program to transition patients from pediatric to adult services while providing quality comprehensive SCD care.
  6. Discuss the role and responsibilities of a sickle cell disease medical care team with emphasis on the newer emerging expanded roles of social workers and the inclusion of patient navigators.
  7. Discuss the impact of racism, complicit and implicit bias, and health disparities on the health of persons living with sickle cell disease.
  8. Describe the U.S. health care delivery system as it relates to SCD care and identify the role of the National Academics of Sciences, Engineering and Medicine consensus report, “Addressing Sickle Cell Disease A Strategic Plan and Blueprint For Action.”
  9. Identify the principles of the Health Insurance Portability and Accountability Act (HIPAA) requirements, confidentiality and ethical boundaries between patients and health care team members as it relates to SCD health care.
  10. Identify effective communication using techniques such as motivational interviewing based upon knowledge of communication theories such as adult learning theory, health belief model and the transtheoretical model.
  11. Discuss the patient perspective of SCD care and how the medical care team can support patients’ efforts to reach optimum health within limitations of living with a chronic disease such as SCD.

 24.25 CE credits will be awarded for psychologists attending the entire program. Continuing Education (CE) credits for
psychologists are provided through the co-sponsorship of the American Psychological Association (APA) Office of
Continuing Education in Psychology (CEP). The APA CEP Office maintains responsibly for the content of the programs.

Complaint Policy and Resolution Process
All complaints must be made in writing to [email protected].

All complaints will be logged into the complaint log. The Director, Continuing Education will attempt to resolve the complaint within the first contact, when possible, and will respond to the client and customer's concerns within 5 business days of the initial complaint. At the time of the first contact, Director, Continuing Education will inform the complainant if more time will be needed to research the complaint.

If the complaint is still unresolved after communication with the Director, Continuing Education, or if the initial complaint is not addressed in a timely manner, the complainant may contact the President & CEO, UHS-PEP. The President & CEO will then contact the complainant and endeavor to resolve the complaint within 10 business days after being contacted.

The complaint log will be regularly reviewed to identify areas of operations and service that may require improvement. The Director, Continuing Education will research reoccurring complaint matters and recommend process and procedure changes, when appropriate. These changes will be noted on the Complaint Resolution Log, and any necessary policy and/or procedure updates will be made within 15 business days thereafter.
